Data Subject Access Requests
For Current and Former Employees of Feel Good Collaborative, or for Customers and Fans of Feel Good Collaborative residing in Europe.
Feel Good Collaborative is aware of its obligations as a data controller with reference to the personal data it manages, processes, and stores, and is responsible for a duty of care for the personal data within its control.
Data subjects whose personal data is held by Feel Good Collaborative are entitled to ask whether the data controller is processing any personal data about that individual and, if so, to be given:
A description of the personal data
The purposes for which they are being processed
Information on any organization to whom that personal data is being, or might be, disclosed
The data subject must also be told about the sources from which Feel Good Collaborative derived the information, so long as those sources are available to Feel Good Collaborative, and for a copy of the information held, in response to a valid request to that effect. The right does not extend to information that are not open to public inspection, for example to provide proof of entitlement to benefits. Feel Good Collaborative will, however, require sufficient details to readily locate the information requested and cannot undertake open-ended searches.
Data Subject Access Request Procedure for Customers and Fans of Feel Good Collaborative residing in Europe.
To see a copy of your own data held by Feel Good Collaborative, you will need to submit a request by faxing the completed form to Feel Good Collaborative, along with multiple forms of current identification, bearing your photograph and your signature, that will allow us to verify your identity and match that identity with our data records, such as a copy of your passport, driver’s license, and other supporting documentation.
This form, and all accompanying pages of supporting documentation, should be notarized before faxing as a means to assist Feel Good Collaborative in accurately verifying the identity of the data subject / requestor.
You are encouraged to complete the Data Subject Access Request Form, with as much relevant detail as possible, to assist Feel Good Collaborative in accurately identifying the location(s) of your personal data. However, any notarized request in writing that is submitted via fax (as specified in the form) is acceptable. Feel Good Collaborative will respond to your request as quickly as possible, and without undue delay, once we have received all of your necessary information to process your request. The more detail you’re able to provide concerning your request, the more likely Feel Good Collaborative will be to find the information that you seek.
Please note that the right to access does not apply in a number of cases, in order to strike a balance between the rights of the individual and the operational requirements of Feel Good Collaborative. For this reason, there are circumstances under which Feel Good Collaborative will withhold certain data from disclosure. These circumstances will be reviewed on a case-by-case basis.

California residents have the right to submit a request to Access, Delete or a request disclosure of the categories and specific pieces of personal information that Feel Good Collaborative has collected.
Data subjects whose personal data is held by Feel Good Collaborative are entitled to ask whether the data controller is processing any personal data about that individual and, if so, to be given:
A description of the categories and specific pieces of personal information collected
The purposes for which they are being processed
Information on any organization to whom that personal data is being, or might be, disclosed

When Feel Good Collaborative receives a verifiable consumer request from a California Resident to delete their personal information, Feel Good Collaborative will delete the consumer’s personal information from its records and direct any service providers to delete the consumer’s personal information from their records.
Response Timing and Format. We aim to respond to a consumer request for access or deletion within 45 days of receiving that request. If we require more time, we will inform you of the reason and extension period in writing.
